Making an Appointment

Most often, your primary care physician will refer you to Sleep Management Institute by sending us your contact information. We will then contact you within 48 hours to schedule your initial office visit.

To make an appointment without a referral from your primary care physician, simply call us.

Once your appointment is made, we’ll need some additional information completed prior to your arrival. Preferentially, you may complete these forms electronically; otherwise, we will mail you a Patient Packet to complete and bring to your consultation. Your responses in the Patient Packet will provide important information for your comprehensive evaluation by our Sleep Practitioners.

Although our services are covered by all major insurances, this is a good time to learn more about your specific insurance coverage relating to sleep disorder diagnosis and treatment. Our staff can help you through this process.

Comprehensive Consultation

During your in-depth consultation, one of our practitioners will spend time listening to you to understand your problem and concerns. A determination will be made whether a sleep study is needed for diagnosis.

Cancellation Policy

Please note that once you have booked an appointment with us it means that we have reserved time in our schedule exclusively for you. Sleep Management Institute is committed to providing exceptional care. Unfortunately, when one patient cancels without giving enough notice, they prevent another patient from being seen. If you cancel your appointment less than 24 hours before it is scheduled to take place, or if you do not show up to your scheduled appointment, you will be subject to a cancellation/no show fee of $100 for a procedural appointment and $50 for an office visit. This fee will need to be paid before you are allowed to reschedule an appointment. These fees are your personal responsibility and are not billed or paid by your insurance company.
